Residential Services

t.l.c. home is an (ICFMR) Intermediate Care Facility for profoundly mentally retarded persons who are non-ambulatory with multiple disabilities.

The purpose of the t.l.c. home is to provide the substantially mentally retarded persons with:

  1. long-term, continuing, educational, therapeutic, medical and 24-hour nursing care;
  2. self-help, social, recreational and personal growth opportunities through an individualized program utilizing specially trained and skilled staff.
  3. opportunity to participate to the extent possible in recreational, social, civic, and other appropriate community services and/or program; and
  4. to provide parents and family members of said persons the opportunity for close interaction with resident and all phases of services.

The area MH/SA/MRDD Interagency Council portal performs admissions to the home. The Interagency Council determines the needs of the individual based on the t.l.c. home's admission criteria.

Individual Habilitative Plan

Trained staff implement habilitative, behavioral, and health care programs developed by a team of professionals and individualized to address each person's strengths and needs. These programs are designed to foster attainment of independent living skills, increase positive behaviors, maintain optimal health and safety, and enrich the lives of individuals.

The Habilitative Team includes a R.N., psychologist, physical therapist, speech therapist, occupational therapist, dietitian, home manager, Q.M.R.P. (Qualified Mental retardation Professional), Executive Director, the parents of guardians, and in some cases, the person being served. Other specialist will be involved as needed. This team creates the annual plan and the person's programs are monitored monthly through observation and data collection. Changes tot he annual plan are done as needed.

Community based services

The folks we serve participate in a wide variety of activities that foster community inclusion.

The school age children attend public school at Floyd L. Knight School/The Children's Center located at 607 McIntosh Street in Sanford, N.C.

The young adults are involved in an Adult Day Program on the t.l.c. home grounds.

The Q.M.R.P. coordinates the in-house and community programs to ensure continuity of active treatment.

The residents of the t.l.c. home participate in Special Olympics, church services, community recreation and entertainment.

 

Outings

Community outings include trips to circus, Lee County and N.C. State Fair, library, bowling, shopping, area parks, school dances and activities.

The Stevens Foundation is a non-profit agency that provides services and programs to people with cognitive, developmental and physical disabilities.

The folks we serve at the t.l.c. home often attend social functions at the Stevens Foundation center.

Another special outing is a trip to Cary, N.C. for a fun evening at the Coffee House. Sponsored by the Resurrection Lutheran Church. The Coffee House provides entertainment and refreshments to children, adolescents and adults with special needs and their families every 2nd Friday of the month. There is no charge. The Coffee House accepts donations to cover costs and organizers recruit entertainers who are interested in donating their time and talent for this rewarding project.   Eric joined the t.l.c. family in October of 2005.

When not at home socializing with friends, Eric enjoys the movies, swimming, bowling, and going out to eat.

A couple of his favorite TV. shows are "reality shows" such as, "Your momma's so fat" and the Donald Trump show.

Eric has been known to inform different staff, "You're Fired".

Welcome, Daniel!

Charming Daniel relocated to the t.l.c. home in August of 2007.

His sense of humor puts a smile on everyone's face.

Daniel has many interests. From playing his drums to sports video games, Daniel packs a lot into a day.
